Documents Needed for Student Registration at Jeffries School

• Proof of Residency in Jeffries School Boundaries (lease agreement, city utilities bill for that address, phone bill, contract on house, etc.)
• Picture ID of parent registering student
• Student's Immunization Record
• Student's Legal Birth Certificate
• Student's Social Security Card

If student has an IEP and/or a diagnostic summary we must have a current copy.

All enrollment forms must be filled out completely, front and back of each form.
