Obtaining a Certified Copy of Your Birth Certificate

Option 1:

Visit the Springfield-Greene County Health Department at 227 East Chestnut Expressway
Fill out an application form
Show a photo ID
Pay $15.00 per certificate with credit card, cash, check or money order (to City of Springfield)
Receive a certified copy of your birth certificate within minutes

Option 2:

Call the Springfield-Greene County Health Department at (417) 864-1411
For $22.00, order a certified copy of your birth certificate over the phone using a major credit card
Receive your certified copy via US mail within a week

Option 3:

Mail a completed birth certificate application form along with a check or money order for $15.00 (made out to the City of Springfield) to:

Springfield, MO 65802

Please do not send cash through the mail
Receive your certified copy via US mail within a week

Option 4:

Fax a completed application form to the Springfield-Greene County Health Dept. at (417) 864-2065
Pay $22.00 per certificate with your major credit card (please provide your credit card number on the application)
Receive your certified copy via US mail within a week

Option 5:

Go online to www.vitalchek.com and order a birth certificate for $22.00 using a major credit card
Receive your certified copy via US mail in about one week

PLEASE NOTE:
The Springfield-Greene County Health Dept. can provide certified copies of birth certificates only for individuals born in Missouri between 1920 and present. The office is open Monday through Friday 8:00am – 4:30pm.

If you were born outside Missouri, go online to www.vitalchek.com to order a certified copy of your birth certificate with your major credit card, or call (417) 864-1411 for the phone number of a health department in the state in which you were born.
