ALL physicals MUST be completed on the new form. No other forms will be accepted. Pick up new forms at the GHS Main Office or download here.

Varsity Games:
9th Grade to Adult - $4.00
Kindergarten - 8th Grade - $2.00

Middle School, Freshman, Junior Varsity Games:
9th Grade to adult - $3.00

What is an ATC?

As a key member of your high school athletics program, St. John’s certified athletic trainers (ATC) are highly educated and skilled health care professionals. Working in cooperation with physicians, school athletics administrators, coaches and parents, your ATC will be onsite to help prevent, assess, treat, and provide basic rehabilitation of injuries.

St. John’s Sports Medicine athletic trainers provide your school with 25-30 hours of practice and event coverage each week. In the event an athlete is injured during practice or a game, the ATC will be there to evaluate the injury, provide immediate care when needed, and communicate directly with you.

Head Injuries

A head injury can increase in severity at any time; therefore an athlete with a concussion should not be left alone. St. John’s Sports Medicine requires all head injuries be evaluated by a physician. Clearance from a physician must be obtained before the athlete is allowed to return to play.

The primary purpose of Ozark Conference Interscholastic Athletics is to promote the physical, mental, moral, and emotional well-being and development of all involved through the medium of contests.

OZARK CONFERENCE
SPORTSMANSHIP STANDARDS

Ozark Conference players, coaches, and spectators will . . .

· regard game officials as fair, accept their decisions as final, and treat officials with respect.
· respect the property of our school and other schools.
· cheer for their team.
· treat the opponent’s coaches, players, and spectators with respect.
· show appreciation of good play by both teams.
· accept the decisions and judgments of the coaches.
· wear t-shirts/clothing that displays positive language.
· remain off the playing field/court before, during, and after events.
· be modest in victory and gracious in defeat.
· maintain an atmosphere of good sportsmanship at all times.

Ozark Conference players, coaches, and spectators will refrain from . . .

· Actions that will offend, embarrass, or intimidate any individual athlete.
· Actions that will call attention to yourself.
· Taunts, chants, noises, cheers, jeers, songs, profanity or motions directed at the opposing team, coach, or school.
· Using noisemakers
· Throwing objects.
· Displaying signs of any type other than official school banners.

Failure to comply with Ozark Conference Sportsmanship Standards may result in your removal from the site of competition and may result in further disciplinary actions.
