Springfield Youth Soccer
is organized through Lake Country Soccer. You can register either
at their website, or call 862-3211.
Prior to the beginning of
participation in any sport, students must have (1) paid participation
fee, (2) received physical examinations from licensed physicians,
and (3) submitted a completed physical examination form to their
coaches. Physical examination forms are available for download here (front)
and here (back)
or can be picked up in the office to take with you to the doctor
for your physical. Coaches will be distributing a Citizenship
Standards for Eligibility card to each student. This card must
be signed and returned to the coaches the following day of practice.
