Football Information Cherokee Middle School Football Camp Monday August 6 and Tuesday August 7 8:00 AM to 12:00 Noon Highly recommended for 7th and 8th grade football players, but is not mandatory for participation in football at CMS Players should come to camp dressed in shorts, t-shirt, and cleats The cost is $40.00 which includes a t-shirt for each camper Applications will be taken up to the first day of Camp, but would be appreciated by August 1st For questions, call Coach Byrne 887-5709, please do not call the school See a coach or the main office for an application Physical and Paperwork are due August 6 ____________________________________________ Paperwork & Fees 1). Physical examination form - must be returned before a player can practice!!! 2). Player medical history - parent permission and authorization for treatment - student agreement regarding conditions of participation must have signatures of parent and player plus insurance company name and policy number!! 3). Participation fee card - must be purchased in the main office for $50.00. Fee covers all sports at Cherokee and includes a student pass for all R-12 athletic events. 4). Coaches will be distributing a Citizenship Standards for Eligibility card to each student. This card must be signed and returned to the coaches the following day of practice. 5). Only transfer and new students may participate after school has begun.
