2012 Girls' Basketball Information

Prior to the beginning of participation in any sport, students must have (1) paid participation fee, (2) received physical examinations from licensed physicians, and (3) submitted a completed physical examination form to their coaches. Physical examination forms are available for download here or can be picked up in the office to take with you to the doctor for your physical. Coaches will be distributing a Citizenship Standards for Eligibility card to each student. This card must be signed and returned to the coaches the following day of practice.

Parents: Please fill out the Student Health Inventory Form and return with student(s). Forms can be downloaded here or can be picked up in the office.

All paperwork and fees must be turned into the coach to practice.
Appropriate clothing for the first few days of football workouts is gym clothes and tennis shoes.
