New MSHSAA Sports Physical Form for the
2011-2012 school year

ALL physicals MUST be completed on the new form. No other forms will be accepted. Pick up new forms at the Cherokee School Main Office or download here.

Athletic Participation Cards

Students who plan to participate in our athletic program will need to make arrangements to pay a participation fee. The athletic participation fee is $50.00 this year (subject to change). The participation fee must be paid before the first game in order for the student to be eligible to play. Students paying the athletic participation fee will receive an activity pass which will entitle them to free admission to all Springfield Public School athletic events for the current school year.

Activity Cards

Adult activity cards will be available in the Cherokee Office. The cost of the card will be $15.00 (subject to change). The adult passes are for admission to only Cherokee football, basketball, volleyball and track events.

An activity card will be available for $5.00 (subject to change) per student. The ticket will provide admission to all Cherokee football, basketball, volleyball and track events and Kickapoo High School games. You will not need this card if you purchase the Athletic Participation card. This card does not discount the Athletic Participation card. Passes are not good for end of season tournaments.
