Students participating in activities for the 2011-2012 school year will need to use the new physical form from MSHSAA. One section of the form requires a signature of acknowledgement from the parent/guardian that they have received and read the MSHSAA packet on concussions. The physical form and link to the concussion information is posted on the SPS Athletics website.
