Forms for Enrollment

 

Springfield Public Schools has a standard enrollment form that is filled out upon entry to a school. Parents are also asked to fill out an enrollment card which has emergency contact information and lists all individuals that have permission to pick up a student.

Papers Needed to Bring to Enroll

 

Students are eligible to enroll for school in the year they reach age 5 by August 1st. Pre-Enrollment days for incoming kindergarten students are held at each Springfield elementary school in mid-March. New student enrollment for all grade levels at each Springfield school begins in early August and continues through the start of school.

 

Parents/Guardians should bring the following paper with them to enroll a student at any school:

 

Birth Certificate

Social Security Card

Immunization Record

Proof of Residency (such as a utility bill mailed to the home)

 

Copies will be made of these documents at the time of enrollment.
